(2) Brief-Copy Separation failure: several of my brief's meta-instructions ('Watch-closed item', "an update on this morning's item", 'Competing-figure note for the colophon', and similar) were written INSIDE the quoted item prose, so the writer faithfully printed them; they were caught in the pass loop, not by the Wake-96 Clean-Copy Gate (whose token list lacked them).
